The Open Account Form, Web Enable Account Request Form, and Tax Resale Certificate, below, MUST ALL be completed!
You will need Adobe's Acrobat Reader to download the forms required. You must print them then complete them by hand. Once completed, they MUST be signed by an authorized agent of your company!

| Step 2. Downloading Forms |
| Once you have Acrobat installed, you may click on any of the forms below. When you click on a form, Adobe Acrobat will launch within your current web browser. |

| Step 3. Faxing Back Forms |
Once you have completed the necessary forms and obtained authorized signatures, please fax them to the Credit Department's dedicated fax number 210-704-8319 along with a copy of your Tax Resale Certificate. This will expediate processing of your account.

Finally, mail all three of the original forms to:

Southwest Wholesale Supply will begin processing your account based on the faxed copies, but the originals are required before an account will be opened.
